-Invoices are sent out on the 1st of each month to your email. -Tuition is not reduced for months with holidays, vacation weeks, or absences due to illness. In the event of an extended illness, please contact the studio to discuss possible arrangements. -Accepted Payment Methods: Cash, check, or online payments. All checks should be made payable to Melrose Dance Academy. -If payment is not received by the due date, a $25.00 late fee will be added to your account for each month the payment is late. -There will be a $25.00 fee for any returned payments. -Monthly tuition is never pro-rated, regardless of how many classes are attended in a given month. -All registration fees and tuition are non-refundable and non-transferable. -If a student’s account is two months behind, the student will not be allowed to attend class until the account is brought current.

-A 30-day written notice is required to withdraw from classes, submitted via email to MelroseDanceAcademy@gmail.com. -Tuition for the upcoming month will be charged unless the written notice is received at least 30 days before the 1st of the month. -No prorating of tuition. Full payment for the month is due if withdrawal notice is not received prior to the start of the month. -Withdrawals after February 1st will incur a 50% charge of remaining tuition.

Students may make up missed classes due to emergencies, health-related absences, or family vacations. To arrange a make-up, email a doctor's note with the missed class date within 1 week. -Dancers may attend up to 5 make-up classes per season. -Make-ups must be within 2 weeks of the missed class and in a similar style/level. --Recital choreography classes (March-May) cannot be used for make-ups. -Classes missed for other reasons or outside of the time frames are not eligible for make-ups or refunds.

In consideration of being permitted to participate in activities organized or facilitated by Melrose Dance Academy, I, acknowledge and agree to the following: -I understand and acknowledge that participating in dance activities involves inherent risks, including but not limited to the risk of injury, illness, or accidents, which may result from my own actions, the actions of others, or the conditions of the premises. -I voluntarily assume all risks associated with participating in Melrose Dance Academy activities, including but not limited to the risk of bodily injury, illness, or property damage, and I hereby release and discharge Melrose Dance Academy, its directors, employees, agents, and representatives from any and all liability for any injury, illness, or accident that may occur during my participation in Melrose Dance Academy activities. -I understand that Melrose Dance Academy does not provide medical insurance coverage for participants, and I agree to be solely responsible for any medical expenses incurred as a result of any injury, illness, or accident that may occur during my participation in Melrose Dance Academy activities. -I agree to indemnify and hold harmless Melrose Dance Academy, its directors, employees, agents, and representatives from any and all claims, liabilities, damages, losses, or expenses, including attorney fees, arising out of or related to my participation in Melrose Dance Academy activities.

In the event you are unable to reach me, in the case of accident or injury, I give my permission for treatment as deemed necessary by Melrose Dance Academy staff or emergency personnel. I also release Melrose Dance Academy and staff of liability in case of injury or accident.
